About the Heartline Classic
The Heartline Classic is an inaugural charity golf outing benefiting Center for the Heart, a New Jersey-based nonprofit providing compassionate, non-clinical support for individuals and families navigating serious illness, Alzheimer’s disease and dementia, caregiving, end-of-life transitions, and grief.
This event honors Jonathan Strelzik’s ongoing journey with Alzheimer’s and the community of family, friends, colleagues, and loved ones who continue to surround him and Holly.
Golf has long been one of Jonathan’s gathering places: a space for friendship, mentorship, conversation, and connection. The Heartline Classic carries that spirit forward by bringing people together in support of families facing some of life’s hardest passages.
What began as one family’s experience now helps create support for many families navigating Alzheimer’s, dementia, caregiving, serious illness, end-of-life transitions, and grief.
Every person will face death.
Every family will face grief.
Yet we have not built the systems to support either…
C4TH exists to change that—and our donors are the ones making it possible.
Make an impact at the individual level while also catalyzing social change through end-of-life and grief literacy. Event Details

What Your Support Makes Possible
Your participation helps Center for the Heart continue building compassionate, non-clinical support for individuals and families navigating serious illness, Alzheimer’s and dementia, caregiving, end-of-life transitions, and grief.
Funds raised through the Heartline Classic support both the event and Center for the Heart’s year-round mission. Contributions help sustain direct support offerings and the infrastructure required to deliver them responsibly, ethically, and with care.
Expanded Access to Support
Your gift helps make support more accessible to individuals and families who may otherwise be unable to pay.
Planning before Crisis
Support helps create space for planning conversations before families are in crisis.
Grief Support and Education
Contributions help sustain structured grief support, grief literacy, and educational programming.
Alzheimer’s and Dementia-Related Family Support
Support helps families navigating cognitive change, caregiving, anticipatory grief, planning, and evolving needs over time.
Caregiver Support
Your participation helps Center for the Heart support caregivers with education, guidance, community, and compassionate presence.
Community Workshops and Literacy
Funds help Center for the Heart provide community workshops and initiatives that increase death, grief, caregiving, and end-of-life literacy.
This work exists in the space between medical care and lived experience: where families often need guidance, language, emotional steadiness, planning support, and compassionate presence.
Why This Work Matters
Alzheimer’s and dementia do not affect only the person diagnosed. They reshape the lives of spouses, children, friends, caregivers, and entire families.
Families navigating serious illness, cognitive change, caregiving, end-of-life transitions, and grief often face emotional, practical, and relational challenges that clinical care alone is not designed to address.
For many families, support is difficult to find and afford. Center for the Heart helps individuals and families receive support earlier, move through difficult conversations with more clarity and preparation, and feel less alone as they navigate caregiving, serious illness, end-of-life transitions, and grief.
Your participation in the Heartline Classic helps make that possible.
